I give permission for my child to participate in the Gordon Grady Cup at Seacourt Tennis Club on 1st of September 2025. I confirm that my child is in good health and fit to participate in the tournament.

I understand that the tournament will follow a handicap singles format and that appropriate safety precautions will be in place. However, I acknowledge that participation in any sport carries a risk of injury, and I accept that Seacourt Tennis Club, its staff, volunteers, and tournament organisers are not liable for any injuries, loss, or damage sustained during the event, except where caused by proven negligence.

I authorise first aid to be administered to my child in case of an emergency and consent to necessary medical treatment.

I give permission for my child to be photographed or recorded during the event for promotional and archival purposes.
